If you run a nonprofit that needs to collect donations, you’ll want to make sure you have an easy to use donation form.

With Gravity Forms, you can build the perfect donation form in just a few minutes.

From within the form editor, I’ll start by adding a name and email field to collect the user’s contact information.

For this form, I’m going to set my fields to be required to ensure everything is completed before the form can be submitted.

I’m going to add a Product field, change the field label to ‘Donation Amount’ and set the field type to Radio Buttons.

I can use the Product field to collect the user’s donation amount.

If the user selects ‘Other’, I want an additional product field to appear that lets them enter a custom amount.

First, I’ll set the Product field type to ‘User Defined Price,’ which will let users enter the amount they wish to donate.

Next, I’m going to use conditional logic to have this field appear only if the user’s response to the first Product field is ‘Other’.

I’ll next add a ‘Total’ field so that the user can verify the amount they are donating before submitting this form.

Finally, I will need a field to collect the user’s payment information.

For this form, I am going to use the Stripe Card field to collect the user’s credit card information.

I’ll also need to set up a Stripe feed so that I can process one time or recurring payments.

Once I’m done, I’m going to check out my form on a live page.
