The Gravity Forms Survey feature gives you purpose-built tools for gathering and understanding feedback, right from your WordPress website.
Build surveys using field types made specifically for this job – Likert scales, ranking, rating, checkboxes, dropdowns, and text fields – then add conditional logic, set required fields, and customize labels and choices to match exactly what you want to learn.
Once responses come in, view them broken down by question and filter by date or other criteria to spot trends quickly.

- Removed the ability to place a Survey field inside of a repeater field.
- Updated get_value_entry_detail() of GF_Field_Likert, GF_Field_Rank, and GF_Field_Rating for compatibility with the change to GF_Field::get_value_entry_detail() in Gravity Forms 2.9.29.
- Updated the Survey field for compatibility with the state validation enhancements in Gravity Forms 3.0.
- Updated Likert settings so that the Enable Scoring feature is in line with the tooltip.
- Fixed an issue where Survey icon is not displayed on Uninstall screen.
- Fixed a fatal error that occurs when the site is using plain permalinks.
- Fixed an issue where Likert fields could have incorrect styling in RTL languages.
- Fixed a fatal error due to class name duplication.
If you are an Elite or Nonprofit Gravity Forms license holder, you can update to Survey 4.2.3 in your WordPress dashboard, or download it manually from your Gravity account.
Note: After updating, clear your site’s cache to ensure you’re running the latest version. We also recommend excluding pages that include Gravity Forms from caching and script optimization, as cached form pages can interfere with dynamic population, field validation, honeypot, and add-ons that rely on time-sensitive tokens.
