If approved to purchase, your Gravity Forms Nonprofit license must be purchased from a Gravity Forms account with an email address of the same domain as specified on this application.
Only organizations with federal non-profit agency tax status will be considered for a discount. Please attach a current copy of the most recent IRS Affirmation or Determination Letter issued to your organization. If you have questions regarding the tax status of your organization or if you need a current IRS Affirmation Letter, please contact the IRS Call Center at (877) 829-5500.
If your organization is a subordinate covered under an IRS Group 501(c)(3) Ruling, you will need to obtain a letter from the central organization containing their GEN and stating that your organization is covered under their IRS Group-Ruling. To determine if your organization is covered under an IRS Group-Ruling, contact the IRS Call Center at (877) 829-5500.
Organizations outside the United States may also apply for a discount. If your organization does not have a U.S. non-profit status or U.S. fiscal sponsor, an affidavit or proof that the organization is the equivalent of a public charity under U.S. tax law is required. You must attach a recent proof/documentation of charitable status (examples: articles of incorporation, charter, certificate of registered charity). Submissions without proof of charitable status will not be considered.
